relax the grip small forward shaft lean and just let it go. don't stand over the ball to long and make sure your routine is the same. i do the two practice swing two look method. i take two practice swings to get the feel of the putter head. i step over the ball look at my target. look back at the ball look at my target and by the time i focus back on my ball I've started the back swing and commit to a smooth putt. even if i miss as long as the ball went on the line i wanted then i know its just my line i chose that was wrong and not my swing.

Constant grip pressure through the entire stroke and think to yourself it is not a big deal, 1 Billion people could care less if you make it or don't. Don't put to much pressure on yourself it is just a putt..

Google "The Quiet Eye"... it's an article that gives some good insight and science behind putting. Don't look at things too long or you will surely miss. It's about routine and trust. Practice your pre-putt routine and get in a habit that helps make you think less and relax.
